MAJOR MEDICAL INSURANCE: (full & part-time employee) Employee contribution required (40%) of premium. P.I. pays (60%) $0 Deductible Office Visits $20.00 co-pay, then 80%-100% of eligible expenses.

(STD) SHORT TERM DISABILITY: Up to 26 weeks at 66-2/3% of pay for non-work related illness
VISION: One eye exam /year for covered employee and dependents, and one time 50% on VSP's Altair eye wear frames
